Coming in for a consultation?
You will need:
1. Current radiographs (if you have them).
2. List of patient's current medications and allergies.
3. Knowledge of patient's medical history.
You can download our Admission Form and Patient Questionnaire to fill out before your consultation appointment, or plan to come 5-10 minutes early to fill them out.
